This is to inform you of the upcoming Immunization Campaign targeting Grade 9 students for the 2013-2014 school year. All grade 9 students are eligible for a vaccine against meningococcal infection. This vaccine offers protection against four types of meningococcal disease: A, C, Y and W-135. You will be receive a pamphlet shortly that will include the consent form for your child. Please complete both sections of the consent form and return to your child’s homeroom teacher by November 19th. The immunization clinic will be held in the MVHS cafeteria on Thursday, November 21st. Vaccines will not be provided without parental signature on the immunization consent form.
